Adding instructions

A great survey questionnaire or appraisal form will include instructions and examples that help people understand the questions and interpret the rating scale. We recommend adding instructions at the top of the questionnaire to provide some context so that people completing the questionnaire understand the process and how their responses will be used.


You can add examples and instructions within any section by adding a text block and typing your instructions inside.


To add a text block, use the "+" button and then select the "Text Block" item.



You can then type your instructions. In this case, we're going to add the company values so they are highlighted at the very top of the questionnaire:



Within the dialog, you will be able to select whether or not you want the text block to be included in the final report of the 360 or appraisal.


You may even create a section at the top of your questionnaire that will only contain instructions. Once you've added that, you can use the "Save & Preview" button to see what the form will look like. Here's an example of that showing a section called "Company Values" at the top:
